A 29-year-old software engineer attending via Zoom presented with chronic task initiation difficulty, time blindness, and shame after missed deadlines, on a background of previously identified ADHD. He was under GP care; hypnotherapy targeted behavioural and cognitive patterns around starting and sustaining work, not medication decisions. Formulation emphasised External Locus of Control ('I only work when pressure hits'), All-or-Nothing task framing, and Future Avoidance. Three sessions of Utilisation and Skill Building reduced SUDS from 7 to 3 and DASS-21 Stress from 17 to 7. The case shows how hypnotherapy can support adult ADHD management alongside existing medical care.

The Challenge

The challenge was building reliable initiation without relying solely on deadline adrenaline, while avoiding perfectionistic plans that collapsed after one miss. Suggestions had to be concrete, short-cycle, and compatible with ADHD-friendly pacing.

The Process

Session One used Utilisation of his 'debugging' mindset: treat starting as a two-minute experiment rather than a finished product. Hypnotic rehearsal paired a physical cue (open laptop, one sip of water) with beginning the smallest visible next action.

Session Two built Skill Building around time boxing, body doubling imagery, and interrupting shame after a missed block. Session Three rehearsed stakeholder updates and end-of-day shutdown rituals, with an audio for morning initiation practice.

The Result

Measured change:

MeasureBaselineMid-TreatmentPost-Treatment% Change
SUDS (initiation distress)75357%
DASS-21 (Anxiety)107460%
DASS-21 (Stress)1712759%

He shipped two backlog items before midday across consecutive days and sent a proactive status update. Client: 'The wall is still there sometimes, but I have a door in it now.'

Introduction

Adults with ADHD frequently seek help for execution gaps—starting, prioritising, and finishing—despite understanding what needs to be done. Hypnotherapy can complement medical and coaching approaches by installing initiation cues, reducing shame spirals, and rehearsing realistic work sequences under calm focus.

This case reports pattern-level change for one client. It does not claim to treat ADHD as a medical condition in isolation, nor does it use clinic-wide outcome percentages.

Case Presentation

A 29-year-old male engineer described sitting for hours before 'urgent mode' kicked in, then working late with high stress. He reported: 'I know the ticket is small. My brain treats it like a wall until the deadline screams.'

Baseline: DASS-21 Anxiety 10, Stress 17, DAS 138, SUDS 7 for initiation distress. He used a task manager inconsistently and avoided progress updates with his lead.

Formulation: External Locus (motivation as something that happens to him), Magnifying first-step difficulty, and shame-based Global Attribution after slips. Strengths included deep technical problem-solving once engaged and strong interest-based focus.

Discussion

Utilisation made suggestions feel native to his engineering identity; Skill Building created repeatable initiation micro-habits. Hypnotherapy here functioned as a pattern and state-training adjunct to existing ADHD care.

Scores reflect this client's five-week window only.

Conclusion

Three Zoom sessions of Utilisation and Skill Building reduced initiation distress and stress scores for an adult with ADHD-related execution patterns. Concrete cues and between-session audio supported transfer into the workday.
