Does hypnotherapy work for smoking cessation?

The short answer from Cochrane 2019: we cannot say hypnotherapy beats other stop-smoking support or no help. If it helps, the extra benefit looks small.

The short answer is not a yes that marketing can print. Cochrane 2019 reviewed 14 randomised trials (1926 people) with follow-up of at least six months. The authors could not conclude that hypnotherapy is more effective than other behavioural support or unassisted quitting. See the hub for the Grade B frame.

Key evidence

The plain-language summary is the public sentence: there is no clear evidence that hypnotherapy is better than other approaches. If a benefit is present, it is small at most. Certainty was low to very low because many trials were small or poorly designed.

Time-matched behavioural comparisons (six trials, 957 people) did not show a difference. Longer counselling comparisons (two trials, 269 people) did not show a difference. The one no-treatment comparison (40 people) favoured hypnotherapy and is too weak to carry the field.

Adding hypnotherapy to an existing package produced a statistically significant signal in five trials. Cochrane immediately qualifies that: four of the five were at high risk of bias, and heterogeneity was substantial. That is not a licence to advertise a proven add-on.

Carmody 2008 sits inside this review. It is a two-session, patch-plus-hypnosis versus patch-plus-counselling trial. Confidence intervals for the main quit rates include 1.0.

Older pooled estimates such as Viswesvaran and Schmidt 1992 are not a replacement for Cochrane’s RCT filter.

Limitations

People cannot be blinded to receiving hypnosis. Self-reported abstinence overstates success when biochemistry is missing. Allegiance and clinic-origin trials tend to look stronger. Protocol labels hide different doses and scripts.

What we don't know yet

Whether any one script, session count, or combination with varenicline would survive a large independent trial is unanswered. See open questions.
