Intake forms explained

Norwest Wellbeing uses adult, child and teen intake forms: a full intake for new clients, a short check-in for returning clients, and a new-issue form when you switch topics.

There are adult, child and teen versions of our intake forms, and which one you complete depends on who the client is and where you are in your care. New clients complete the full intake; returning clients get a much shorter form.

The form types

  • Full intake, for new clients. This is the detailed questionnaire your practitioner uses to understand your situation and prepare your first session.
  • Short check-in, for returning clients continuing with the same issue. It's about 6 questions covering how things have progressed since last time.
  • New-issue form, for returning clients who want to work on a different issue. It gathers the background on the new topic without repeating everything we already know.

Adult, child and teen versions

Each form comes in an age-appropriate version. Adults complete their own form. For a child, the parent or guardian completes the form on their behalf. Teen forms can be completed by parent and teen together, or the parent can create a link with a PIN so the teen completes it themselves. See teen intake forms.

Why the forms matter

Your answers directly shape your session. Your practitioner reads your form before you meet, so the more accurately you answer, the better prepared they are. For the step-by-step process, see how to complete your intake form.

Common questions

Q: I've been before. Do I have to fill in the long form again? A: No. Returning clients continuing with the same issue complete a short check-in of about 6 questions. You only see a longer form if you're starting work on a new issue.

Q: Who completes the form for my child? A: The parent or guardian completes child intake forms. Teen forms can be done together, or the teen can complete their own via a parent-created PIN link.

Q: When do I get the form? A: After your purchase or booking: a link is emailed to you, and the forms also live in the member portal at members.norwestwellbeing.com.au.

Q: Does my practitioner actually read it? A: Yes. Your practitioner reads your intake before creating your session, and your answers shape what happens in it.
